Membership Cancellation Form

Please complete the below form if you wish to cancel your WV Active membership. A notice period of 30 days before your next Direct Debit payment must be given to cancel your membership. All Direct Debit payments are taken on 1st of each month. For example, if you would like to cancel your membership for February, you must complete the cancellation form by 1st January. Please wait to receive your email confirmation with your last payment date before you cancel your Direct Debit with your Bank.
